What to do if you have a problem or a complaint

If you have any questions, problems or complaints about Care1st, your health care benefits, your doctor or other health care providers call us at (602) 778-1800 or (866) 560-4042, TTY: 711, Monday through Friday, 8:00 am to 5:00 pm, or write to the Care1st Member Services Department at the address listed below. What to do if you need to file an appeal

You or your representative may request a review of an action that denied or limited a service, or where action or services were not timely. Your request may be filed orally by contacting us at (602) 778-1800 or (866) 560-4042, TTY:771, Monday through Friday, 8:00 am to 5:00 pm, or in writing at the address listed below. When sending us a written request, please tell us why you do not agree with the action taken and the action you are requesting.

If you have already submitted an appeal and you want to request a hearing, you must write out your request and send it to us within 120 days of your receipt of our decision letter. Instructions for requesting a hearing are included in our decision letter.
